Forms required before the first day of school are:

Allergy Action Plan
All RPMS Students with allergies or asthma, with required parent's signature

Birth Certificate

• Required for all Toddler students

Medical/Immunization

• Required for all children in Toddler/Preschool, ages 2-5
• Extended Day (Kindergarten)
• 6th graders
• Any new to RPMS Elementary students
