Contracts and Forms

After acceptance into the program, parents sign a contract and submit the following non-refundable fees and deposits:

• Program deposit
• Building Fund Fee (a one-time per family $2000 fee)
• Registration Fee ($130). All fees and deposits are non-refundable.

In addition, the following paperwork must be submitted for children applying for Extended Day (Kindergarten) and Grades 1 through 8:

• Authorization to Release Records Form AuthRelease_Records.doc (44 KB)
• Elementary Application Form Elementary_Application.doc (45 KB) , progress reports from previous schools and student recommendation form (required after above forms are received, to be completed by child's current or previous teachers).
